Pauline Czettel suicide

I found the document detailing Pauline Czettel's suicide. However, the image is too fuzzy and dark to read properly. Also, my German is not very good, so could I ask you for a favour, please? Could you either send me a clearer, enlarged copy - oir if that is not possible, translate for me what is in the document. We would especially like to know her address at time of death and whether she really killed herself. If she has some family reference known, that would be very useful too.
